Acknowledgement of Risk & Waiver

Whilst roller-skating has been a popular leisure activity for many years there is an element of risk inherent. Rollerama requires all roller-skating participants to complete this form and sign. Those participants under the age of 16 years, a guardian must sign on their behalf.

This form will be issued to all participants when they arrive to Rollerama.
